If you have multiple directories on your server, please fill out a survey for each directory.

If you sent all of your payers to Change Healthcare via Revenue Performance Advisor then your User type is RPA.

If you sent only commercial payers to Change Healthcare then your User Type is Non RPA (Emdeon Vision)

Emdeon Vision is being replaced with Optum Intelligent EDI (iEDI). Medent needs to provide Optum with the initial user for iEDI for the account creation. Additional users can be added later. Please only fill out one form per practice. If you already have an OHID, that will be used to log into iEDI but a survey still needs to be completed so that Medent knows to request an Optum account for you.
